Who Can Apply
You have a documented mobility impairment — such as a spinal cord injury, MS, ALS, cerebral palsy, or similar condition — that limits your ability to stand independently.
A physician, physiatrist, or physical therapist has recommended or would support use of a standing wheelchair as part of your care plan.
Your insurance does not fully cover the cost of standing functionality, and the out-of-pocket gap creates a financial barrier to obtaining the device.
How It Works
Complete the form below with your personal information, diagnosis, and financial situation. All information is kept strictly confidential.
Our team reviews your application within 2–3 weeks. We may follow up by email to request supporting documentation such as a physician letter or insurance denial.
If your application advances, we work with you and your provider to understand exactly what the insurance gap is and what device best fits your needs.
Based on available funds and need, we issue a funding decision. If approved, we coordinate payment directly with the device supplier on your behalf.
Your standing wheelchair is ordered and delivered. Our team follows up to ensure everything is working for you.
